4ª = Correctly meant as a signoff in
spades, but interpreted by North as a slam try in spades, since South could have signed
off in spades or NT at his previous turn of bidding. To North, it would not make sense to
investigate 5¨, where 3NT and 4NT would be
easier to bid and make. North, of course, should have obeyed his partner, who had a far
better picture of the slam potential of the NS hands, but he tried:
4NT = RKCB for spades.

East had no trouble finding the §-lead. North
won, played 5 rounds of trumps, and took the inevitable §-finesse...
